Employee's leg amputated after fall into conveyor

At approximately 6:05 p.m. on February 5, 1996, Employee #1 was clearing a bark jam below the hog in the chute above the paddle conveyor at the Weyerhaeuser sawmill in Enumclaw, WA. He walked around to the north end of the conveyor and fell into the unguarded opening at the tail sprocket. Employee #1 left leg was surgically amputated as a result of the accident. Causal factors included poor housekeeping, inadequate lighting in the area, removal of the guard from the tail sprocket area, and lack of a lockout/tagout policy specific to this operation.
