Employee killed in fall from edge of roof

On September 30, 2005, Employee #1 and coworkers were installing a rubber surface to an approximately 4-in-12 pitched roof at a fire station. A safety monitor was being used for fall protection. Employee #1 was using a roller to apply adhesive at the eave edge when he spotted a bottle in the rain gutter. As he reached for it with his roller, the safety monitor yelled at him to get back from the edge. Another coworker yelled, "Let it go for now." Employee #1 stepped back into the fresh adhesive and slipped, falling onto his stomach on the roof. He then slid off the edge and fell 15 ft 8 in., landing on his back and striking his head on the concrete patio below the eave. Employee #1 was killed.
