Employee's finger amputated by chop saw

Employee #1 was using a knee-activated chop saw to cut 5/4 in. by 1 1/2 in. stripping into edging pieces for windows. A piece of wood jammed in the blade slot, stopping the blade from raising through the table. Employee #1 raised the saw's guard and removed the piece. Although the saw was shut off Employee #1 had not locked it out. When he accidentally bumped the knee activator, the blade, which was still coasting, rose through the table and amputated one of his fingers.
