Investigation Summary
Employee #1 had replaced a drive belt. The motor was running but the belt was out of alignment. He attempted to push the belt over with his hand. A rough place on the belt caught his hand and pulled it into the pulley. His finger was cut off by the sharp edge of the pulley. At no time during the repair process was lockout/tagout used.
