Employee dies of broken neck when struck by compressed bale

Employee #1, who was working alone, was making a plastic products bale for recycling. He was using an upstroke Economy baler designed for paper products. The plastic products being baled were sheet or film plastic and foam-type plastic. Employee #1 compressed the bale in the press and opened the two large doors to the press, intending to band the bale, when the bale exploded. It struck Employee #1 and threw him 16 ft from the press. He hit his head on a forklift and broke his neck at the c-3, c-4 level. Employee #1 eventually suffocated and died.
