Employee's foot amputated by mold car

At approximately 12:58 p.m. on September 26, 1991, Employee #1 was removing a marker from one mold car to another. The marker was in a different location. The employee removed the guard on Department line 803 without stopping the production line. He stepped on a bumper and caught his foot, which was amputated between the mold car and the bumper.
