Employee killed when struck by concrete wall panels

On January 29, 1992, Employee #1 and two coworkers were erecting 8 ft by 24 ft 11 in. pre-stressed concrete wall panels. They would set the panel, then anchor the bottom, and then unhook the panels from the crane. At approximately 1:30 p.m., the employees had set three panels. Employee #1 was atop the panels waiting for the welder to finish anchoring the bottom of the third panel. The panels that Employee #1 was on began to tip outward and slowly fall. Employee #1 fell or jumped, and landed in the path of the falling panels. He died from the head injuries he sustained.
