Investigation Summary
Employee #1, a 21-year-old oven operator who had been employed approximately five days at this establishment, was assigned to operate a hand-feed table saw for the first time. His supervisor instructed him on using the saw with an unguarded 10 in. diameter blade. As Employee #1 attempted to complete the run parts, he severely cut fingers on his right hand.
