Employee's fingers amputated by power press

At approximately 10:30 a.m. on or about July 5, 1991, Employee #1 was running a Johnson full-revolution power press, serial #52449, with no anti-repeat, point of operation guards, or hand tools. She placed wire into the die, pressed the palm buttons, and the press cycled. When she removed the piece, the ram came down on her right hand, amputating two fingers. The press malfunctioned due to a faulty micro switch.
