Employee killed by fragment of shattered grinding wheel

At approximately 6:30 p.m. on or about March 1, 1997, Employee #1 was operating a Setco Floorstand snag grinder, serial #64586. The 30 in. diameter abrasive wheel was a Bay State/Sterling reinforced resinoid, #GB5LR-986-1. Employee #1 was standing directly in front of the machine, in the plane of rotation, off-hand grinding a gray iron casting when the abrasive wheel exploded. A 9 in. by 6 in. by 1 5/8 in. piece of the wheel struck Employee #1 over the heart, killing him. The cause of the wheel exploding could not be established with certainty.
