Employee's fingers injured in power press

At approximately 2:15 a.m. on or about September 15, 1994, Employee #1 was placing parts into a Federal full-revolution 23 ton mechanical power press, serial #20210, when he caught his middle and index fingers in the press. The press degloved his middle finger and cut his index finger. The point of operation guarding was a pull-out device.
