Investigation Summary
At approximately 2:40 p.m. on August 31, 1995, Employee #1 was installing sewer pipe in a trench when a wall of the trench collapsed and broke a 6 in. diameter steel gas pipe. He was struck in the head by a flying piece of lumber and killed.
