Investigation Summary
At approximately 9:14 a.m. on August 6, 1992, Employees #1 through #4 were attempting a 4 in. tap on an 8 in. gas main using a 2 in. plugging machine instead of a 4 in. plugging machine. A fire and explosion occurred, burning the employees in varying degrees. Employee #1 suffered 2nd- and 3rd-degree facial burns. Employee #2 suffered 2nd- and 3rd-degree facial and arm burns. Employee #3 suffered 2nd- and 3rd-degree facial, arm, leg, and back burns. Employee #4 suffered 2nd- and 3rd-degree facial, arm, chest and back burns.
