Employee killed when struck in head by pipe cap and hardware

On June 2, 1993, Employee #1, of Statewide Excavating, Inc., was one of five employees installing sewer and water service. He accompanied the foreman to set up equipment to chlorinate the new water main. He installed a bleeder hose on one end and a pipe cap and gas-powered pump to the other end. The pump ran for 20 or 30 minutes, and the foreman told Employee #1 he was going to check the bleeder hose. As the foreman turned he heard a loud pop; Employee #1 was struck in the head by the pipe cap and hardware weighing approximately 40 pounds. He was pronounced dead at Botsford Hospital. The employer received two serious citations.
