On May 21, 1993, Employee #1, of Gerald McCormick Sawmill, was operating a circular head saw. For an undetermined reason he left the control station with the saw still powered and walked to a position adjacent to the saw. He contacted the saw and was decapitated. The employer received a serious citation.
