AN EMPLOYEE WAS WORKING FROM A 6-FOOT WOODEN STEP LADDER INSTALLING LUBRICATION TUBING ON AN AUTOMATIC SPINDLE MACHINE. HE APPARENTLY LOST HIS BALANCE AND FELL INTO AN OPEN 480-VOLT SWITCH BOX. HE WAS ELECTROCUTED.
