Investigation Summary
EMPLOYEE #1 AND ANOTHER EMPLOYEE WERE WORKING FROM A FULLY DECKED WORK PLATFORM LOCATED APPROXIMATELY 20 FEET ABOVE THE GROUND. THEY WERE INSTALLING ANOTHER SET OF 6 FOOT, 6 INCH HIGH WALK-THRU SCAFFOLD FRAMES. EMPLOYEE #1 WAS SETTING THE FOURTH FRAME FROM THE END WHEN APPARENTLY THE LEG CLOSEST TO THE BUILDING CAME OFF THE PIN AS HE WAS ATTEMPTING TO PLACE THE OTHER LEG. HE LOST HIS BALANCE AND FELL. HE WAS KILLED. THE ACCIDENT HAPPENED IN GRATIOT COUNTY.
