Investigation Summary

Investigation Nr: 170210538
Event: 09/22/1992
ELECTRIC SHOCK - DIRECT CONTACT WITH OVERHEAD LINE

A POWER LINE CREW WAS REPLACING A UTILITY POLE. THE EXISTING POLE WAS BROKEN. THE CREW HAD APPARENTLY PLACED THE NEW POLE AND HAD PUT NEW ARMS ON IT. THEY THEN DISCUSSED THE PROCEDURE FOR REMOVING THE TRANSFORMER FROM THE EXISTING POLE. ONE OF THE POWER LINE WORKERS DONNED HIS SAFETY EQUIPMENT AND WENT UP IN AN AERIAL LIFT TO PERFORM THIS OPERATION. HE HAD REMOVED THE TRANSFORMER RISER AND WAS ABOUT TO REMOVE THE 5000-VOLT SWITCH AND LIGHTNING ARRESTER WHEN AN ELECTRIC ARC WAS HEARD. THE POWER LINE WORKER HAD SLUMPED OVER INTO THE AERIAL LIFT BUCKET. THE INJURED EMPLOYEE WAS LOWERED, AND HIS COWORKERS DISCOVERED THAT HE WAS WEARING ONLY LEATHER GLOVES. HE HAD BEEN ELECTROCUTED.
