Investigation Summary
On July 23, 1991, a pneumatic mole had been pushed under the road for a 2 in. conduit. The mole apparently hit a rock and went deeper than the 3 ft depth it was started at. The employees dug a trench 10 to 12 ft long by 6 to 8 ft wide by 10 to 12 ft deep, to retrieve the mole. Employee #1 was on a ladder leaving the trench when the sides caved in. He was killed.
