Investigation Summary
Employees #1 through #3 were pouring a 62 ft by 2 ft by 4 in. thick expansion joint in a parking garage floor when the framework in the double T-joints failed. This resulted in a complete floor joint collapse, which caused the employees to fall 27 ft to the ground. Employees #1 through #3 were hospitalized with multiple fractures.
