Employee falls to death from bridge platform

Employee #1 and two coworkers were on a sand and debris platform that was secured underneath a bridge by hand winches and wire rope cables. They were located approximately 90 ft above a river and a steel barge that was below the bridge. They began to lower the platform using the hand winches. When the winch cables were released, the platform tilted, knocking the employees off balance. Employee #1 tried to help one of his coworkers, who was on the north end of the platform. He grabbed the unstable cable and then a hanging tarp cable, but he could not hold on. Employee #1, who was not using a lifeline, safety belt, or lanyard, slid down the cables until they ran out and fell approximately 30 ft onto the steel barge. He was killed when he sustained massive injuries to his head, neck and upper body.
