EMPLOYEE #1 WAS UNLOADING JAM BLOCK FROM A LIFT TRUCK WHILE STANDING ON A SCAFFOLD 17 FEET ABOVE GROUND LEVEL. THE SCAFFOLD WAS ERECTED ON ROOF TRUSSES. THE PLANKING USED ON THE PLATFORM WAS NOT SCAFFOLD GRADE PLANK. EMPLOYEE #1 DROPPED A JAM BLOCK AND THE SHOCK LOAD CAUSED A STRINGER PLANK. TO BREAK, DROPPING THE SCAFFOLD AND EMPLOYEE TO GROUND.
