Investigation Summary
Employee #1 was attempting to remove a catenary scaffold from the underside of a lift bridge that was in the raised position. Employee #1 fell from the structure and landed at the bottom of the counterweight pit approximately 28 ft below. No fall protection was used. Employee #1 died of a head injury.
