Investigation Summary
While methanol was being pumped into a storage tank from the top, an apparent static charge ignition occurred in the tank, causing deflagration. The ignited vapors traveled to an adjacent tank through a common vent pipe and ignited flammable vapors there, exploding the second tank. Employee #1 was caught in the center of the fireball, sustained third-degree burns to 91 percent of his body, and died. Employee #2 was passing through the area when the fireball struck him on the back, causing third-degree burns to 20 percent of his back.
