Investigation Summary
EMPLOYEE #1, A MATERIALS HELPER, WAS RIDING THE FORKS OF A FORKLIFT TRUCK HELPING PICK UP STACKED MATERIAL AT A HIGH ELEVATION. THE TRUCK OPERATOR SUDDENLY REVERSED THE FORKLIFT, CAUSING EMPLOYEE #1 TO LOSE HIS BALANCE AND FLIP OVER THE CARGO BACK REST AND JAM HIS HEAD INTO THE GRILL OF THE CARGO BACK REST. HE DIED OF A BROKEN NECK.
