EMPLOYEE KILLED DUE TO FALL FROM SKYLIGHT

On July 21, 1997, two employees were working the 3:00 to 11:00 p.m. shift at a tire and rubber manufacturing facility. The employee's were tasked to repair an earth mover tire machine (machine number 66E16) located in plant #1. The chain guards on the assembly did not fit well and the employees went to plant #2 to retrieve new chain guards. The employees decided to go the roof of plant #2 to watch the sunset. Employee #1 sat on the edge of a skylight on the roof of plant #2. He lost his balance, fell through the skylight to the concrete floor inside plant #2, and was killed. Neither employee was trained in fall protection, and no tasks were issued involving accessing the roof area of plant #2. No signs were posted limiting access, but the company did have a written policy regarding roof access.
