Investigation Summary
On October 16, 1999, Employee #1 was trying to clear a jam from the draw side of a can maker, a machine that takes a blank, flat piece of metal and draws it out to make a can. She put her left hand between the draw ring and pressure plate to remove the two jammed blanks. There was an outer guard on the machine which was interlocked, but the pressure on the 12 springs still remained. During normal operation, a hydraulic cylinder pulled the pressure plate back, allowing a blank to enter the die area. The 12 springs held the pressure plate tight against the die. Apparently, Employee #1's removal of the two blanks caused the pressure plate to move, crushing two of her fingers, which had to be surgically amputated. After the accident, the employer installed a rubber block to prevent workers from reaching into the area. Subsequent investigation showed that a hand could still reach over or under the rubber block. Apparently, the company intended to install a different guard large enough to prevent a worker's hand from reaching into this area.
