Employee's fingers amputated in power press

At approximately 10:30 a.m. on March 26, 1998, Employee #1, a temporary worker hired as a press monitor and operator, was clearing away scrap from behind a mechanical power press. He placed one hand on the press to maintain his balance and was picking up a piece of scrap with the other hand when the press operator cycled the press, amputating four of Employee #1's fingers. The operator did not realize that he was behind the press. Although Employee #1 had been hired as a press operator, he had no prior experience other than in the food service industry.
