Investigation Summary
Employee #1 was in a kneeling position inside an extractor (wax spinner) while he was cleaning it. When the machine unexpectedly started to spin, the employee stood up in an attempt to escape and struck his head against the steel lid that was in the open position. Rescue attempts were unsuccessful and he died instantly. Lockout procedures were not used. The reason for the machine restarting has not been determined and an electrician could not duplicate the malfunction.
