Employee killed in fall from roof

At approximately 4:50 p.m. on December 13, 2000, Employee #1 was helping coworkers install a metal roof on an non-tiered building. Two 12 in. wide panels wide had been installed when the workers began to gather and secure the remaining panels on the roof at the end of the day. In the process, Employee #1 lost his balance and fell 26 ft off the roof. He died of head injuries sustained in the fall. Snow had fallen earlier in the day and the roof panels were wet. No fall protection system was in place at the time of the accident.
