Employee killed when caught under descending transfer lift

At approximately 2:30 p.m. on September 7, 2000, Employee #1 was installing electrical conduit under a conveyor system approximately 14 in. to 15 in. away from the vertical transfer lift (VTL) when, because of an approximately 8 in. high obtruding lip, he turned around and put his body under the VTL to connect two pieces of conduit. The VTL descended, trapping Employee #1 under it. He sustained crushing injuries and was flown to the hospital; he underwent several surgeries but died at approximately 12:30 p.m. on September 8, 2000. Employee #1 had worked for the company for approximately three years. The opening of the VTL had not been guarded nor had it been locked out or tagged out of service.
