Event: 09/25/2000
Employee killed when struck by cartons

At approximately 2:15 a.m. on September 25, 2000, Employee #1 was using a Toyota lift truck to stack cartons of lawn and garden tractors. She was placing the uppermost of six cartons when the second carton from the bottom gave way. As Employee #1 backed away, the entire stack of cartons began to collapse. She jumped out of the operator's seat and was struck by the top two cartons, each weighing between 695 lb and 725 lb. She suffered a fractured skull and brain lacerations and was transported to the hospital, where she was pronounced dead at 10:15 a.m. The forklift was equipped with clamps and overhead protection. At the time of the accident, Employee #1 was not wearing a seat belt.
