Employee killed while changing truck tire

At 4:30 p.m. on February 7, 2004, Employee #1 was found lying under a semi truck. The employee had removed the front left tire and was under the truck when the jack apparently had slipped out from under the vehicle. Employee #1 was killed from internal injuries to his upper body.
