Employee drives tractor off roof while hauling debris

At approximately 10:30 a.m. on August 22, 1991, Employee #1 was operating a small tractor on a flat roof hauling debris to the drop-off area from where the debris was to be thrown off the roof. Employee #1 did not disengage the tractor pedal while at the drop area and drove off the 4 foot high roof. Employee #1 received bruises.
