Employee's hand injured in chromojet machine

At about 12:15 a.m. on May 15, 1994, Employee #1, of Bentley Mills, Inc., was installing a new nozzle on a chromojet machine and tightening it with a wrench. When the machine was suddenly activated, it moved, catching Employee #1's right hand between the print head and the print blade. Suffering amputation of middle finger and the end of the fourth finger on his right hand, Employee #1 was hospitalized. Power supply to the machine had not been disconnected and moveable parts had not been blocked against inadvertent movement prior to servicing, cleaning, or adjusting. The power control was located about 20 ft from where Employee #1 was working.
