Investigation Summary

Investigation Nr: 170738694
Event: 06/06/1994
Employee's finger partially amputated by shear

At about 12:30 a.m. on June 6, 1994, Employee #1, of Stanley Hardware, was cutting cardboard on a shear press (#24804) when his right middle finger was caught under one of the plungers of the machine, according to a maintenance lead person. The employee's right middle finger was partially amputated by the workpiece holder. Employee #1 was working alone in the roll form area during the 3rd shift, thus no one actually saw what happened. There were a few employees elsewhere inside the plant when the accident occurred. Employee #1 could not be contacted about the accident so observations were based on information supplied by the company officials.
