Employee's fingers amputated in heat transfer press

At 11:53 p.m. on May 2, 1991, Employee #1 was putting molding plates into a heat transfer press, model #1D15-M159. She pushed the activation button with her left hand before she removed her right hand from the press. Her fingers were crushed and later had to be surgically amputated. The machine was made in Italy and had no point of operation guards. The employer was cited for a serious violation of T8CCR 4184(b).
