Investigation Summary
At 11:53 p.m. on May 2, 1991, Employee #1 was putting molding plates into a heat transfer press, model #1D15-M159. She pushed the activation button with her left hand before she removed her right hand from the press. Her fingers were crushed and later had to be surgically amputated.
