Employee's arms crushed in forklift mechanism

Employee #1, of A1 Body Shop, was prepping a Mitsubishi forklift for painting. The forklift's forks were raised as Employee #1 was masking. The lift moved downward, trapping and crushing both his arms. Employee #1 does not remember the incident and it has not been established how he put both arms through the lift mechanism. There were no witnesses. Certified engineers examined the forklift and found no fault.
