Employee's hand lacerated when caught by router

Employee #1 was a "set-up" man, authorized to set-up woodworking machines for other operators in the shop and to do jigging and related work on his own. It was close to his quitting time, and he elected to skip making a jig fixture to hold down the material he was prototyping since he only needed three of them. The Wadkin Bursgreen router cut away from him, moving much too quickly. During the movement, his hand was drawn into the router and severely lacerated. Employee #1 had extensive training in this type of prototype work, and there was an abundance of documentation relating to his training and qualifications. No defects were found upon inspection of the router/shaper. No point of operation guarding is required or in place for this machine except the complete jig fixture, which isolates a worker's extremities from the point of operation. Employee #1's job responsibilities meant that he was not closely supervised, as were the other workers.
