EMPLOYEE AMPUTATES FINGER IN INJECTION MOLDING MACHINE

AT ABOUT 2:45 PM ON OCTOBER 1, 1990, EMPLOYEE #1, A MACHINE OPERATOR AT CROWN CITY PLATING COMPANY, WAS REMOVING 2 PLASTIC PARTS THAT WERE STUCK IN THE MOLD. SHE OPENED THE SLIDING SAFETY GATE AND STARTED REMOVING THE PARTS. SHE WAS ABOUT TO RELEASE THE 2ND PLASTIC PART WHEN THE EJECTION TIMER RAN OUT, CAUSING THE MOLD TO RETRACT AND PINCH HER RIGHT MIDDLE FINGER. SHE IS STILL RECEIVING TREATMENT UP TO THIS TIME. THE EMPLOYER HAD TO CHANGE THE CIRCUITRY OF THE MACHINE, THE SAFETY SWITCH, AND TIMER SO THAT IT WILL STOP OPERATION IN ORDER TO PREVENT A SIMILAR INJURY. THE EMPLOYER SHALL ALSO PROVIDE A SPECIAL HAND TOOL TO REMOVE ANY PLASTIC PARTS STUCK IN THE MOLD.
