At approximately 5:30 p.m. on November 26, 1991, Employee #1, of Patio Industries, was reaching over a conveyor to grab a 15 lb cement bag when his right hand became caught between the conveyor and steel rollers. Employee #1's right hand was crushed. He was hospitalized.
