Employee's finger amputated by mold

At about 11:00 a.m. on June 6, 1994, Employee #1 was working on a mold (J 601 Rain Bird Mfg., model no. 76052, 4 cavity inner stem, part no. 109671) without using a hand tool. He was trying to put two parts together by pushing one part of the mold to engage the guiding pins with the holes on the other part. As he moved the mold, his left smallest finger was on one of the holes; when a guiding pin hit the hole it amputated his finger. After investigation, the employer was cited for a serious violation of T8CCR 333060.
