Employee's wrist fractured in fall when ladder slides out

At approximately 1:30 p.m. on December 13, 1990, Employee #1, an electrical contractor in Ontario, CA, was on a 20 ft two-section portable metal extension ladder fixing an electrical alarm system for the emergency exit in a warehouse in Pomona, CA. The ladder accidentally slid out, causing Employee #1 to fall about 15 ft to the floor. His right wrist was fractured and he was hospitalized overnight at Pomona Valley Medical Center. There had been a coworker standing below Employee #1, but he had not been holding the ladder when the accident occurred. The ladder was resting against the north wall of the warehouse, near an emergency exit, and was not secured.
