EMPLOYEE INJURED IN FALL FROM LADDER

AT ABOUT 11 A.M. ON OCTOBER 2, 1990, EMPLOYEE #1 OF STAINLESS FIXTURES, INC., FRACTURED HIS RIGHT ARM AFTER HE FELL OFF AN EXTENSION LADDER WHEN IT ACCIDENTALLY SLIPPED FROM POSITION. EMPLOYEE #1 WAS FIXING AN ELECTRICAL CONDUIT ON THE WEST WALL OF THE SHOP, NEAR THE SHUTTER GATE, WHEN THE ACCIDENT OCCURRED. HE WAS USING A 16-FOOT EXTENSION LADDER FOR THE JOB THAT WAS NOT POSITIVELY SECURED AGAINST ACCIDENTAL DISPLACEMENT, AND HE BECAME UNBALANCED AND FELL TO THE CONCRETE FLOOR WHEN THE LADDER SLIPPED. HE WAS HOSPITALIZED FOR ABOUT 3 HOURS AT POMONA VALLEY MEDICAL CENTER AND LATER RELEASED TO RECUPERATE AT HOME. EMPLOYEE #1 COULD NOT BE CONTACTED ABOUT THE ACCIDENT. THE ACCIDENT DETAILS WERE OBTAINED FROM THE SHOP FOREMAN WHO DIDN'T SEE THE ACCIDENT BUT WAS AROUND WHEN IT HAPPENED.
