Investigation Summary
At approximately 1:15 p.m. on May 3, 1991, Employee #1 was setting up risers for a student chorus in an elementary school cafeteria in Irwindale, CA. As she unfolded one of the risers, her left ring finger became caught between the parts and the tip of her finger was amputated. She was treated by paramedics from station #48, and then transported to Inter-Community Medical Center in Covina for further treatment. The investigation revealed a violation T8CCR 342(a): the employer did not report this serious accident to DOSH. Other general and regulatory violations were found and the employer was cited accordingly.
