Employee fractures heels in fall from roof

Employee #1, a foreman at a work site, was installing tar paper on a composition shingle roof with an 8:12 slope. He slipped and fell, then landed on a truck bed. Both of Employee #1's heels were fractured. No roof jacks were installed where Employee #1 was working, and he was not using a safety belt and lifeline. The employer was cited a serious violation for not supplying adequate fall protection.
