EMPLOYEE INJURED WHEN FORKLIFT FORK FELL ON HIS TOE

EMPLOYEE #1 BOUNCED THE FORKS OF A FORK LIFT ON SOME MATERIAL, CAUSING THE CHAIN TO COME OFF THE ELEVATING SPROCKET. IN ATTEMPTING TO REPLACE THE CHAIN WITH A LEVER, HE LIFTED THE SPROCKET OFF ITS SUPPORT. THE FORKS THEN FELL, STRIKING THE EMPLOYEE'S RIGHT BIG TOE. THE EMPLOYEE USED IMPROPER PROCEDURES AND WILL BE DISABLED AS A RESULT. NO ACCIDENT-RELATED VIOLATION WAS ISSUED.
