Investigation Summary

Investigation Nr: 201110723
Event: 12/19/1997
EMPLOYEE INJURED IN FALL FROM LADDER

At 9:00 a.m. on December 19, 1997, Employee #1 of U.S. Sign and Lighting was working alone, installing Christmas lighting at a customer's private residence. Employee #1 had extended the Sponco Manufacturing Inc. ladder truck outriggers and had extended the ladder approximately ten feet. The street fell away to the left at about a five degree slope, and the driveway sloped up at about the same grade. The ladder was elevated about 15 degrees. Employee #1 was climbing the ladder, and was about 75 percent of the way to the top when the ladder started to rapidly rotate counter-clockwise. Employee #1, in order to get clear of the hazardous situation, jumped from the ladder and fell about 12 feet to the street. He suffered serious injury to his right foot and leg. Area residents called the paramedics and he was transported to the hospital.
