Investigation Summary
At approximately 9:45 a.m. on May 17, 1993, Employee #1 was working on a roof at an approximate 9 1/2 ft elevation. He was marking plywood sheeting for cutting and had one knee on top of a stack and the other foot on the open rafters. According to the owner of the company, there was a California bundle (pieces of 2 by 4s) on the roof above Employee #1. The bundle slipped down into the stack of plywood and caused Employee #1 to lose his balance and fall from the roof. He suffered a broken right arm and a fractured disc in his back. Employee #1 was hospitalized from May 17, 1993, to May 25, 1993, at Palmdale General Hospital and had surgery on his arm.
