Employee's finger amputated in punch press

At approximately 12:45 p.m. on January 29, 1991, Employee # 1, a machine operator, was using a full revolution, mechanical, power-operated punch press to stamp out small metal gaskets. He reached in to clear a piece of the thin metal stock that was being struck on the press die. His hand was in the point of operation when the press cycled, partially amputating his finger. Employee #1 did not shut off the power before reaching into the press, nor did he use the available extension grips.
